We've made the intake process as straightforward as possible. Here's everything you need to know before your first appointment.
Use our online form or call us directly. We'll confirm availability and match you with the right provider for your needs.
You'll receive a secure link to complete intake forms before your visit. This typically takes 15–20 minutes and helps your provider prepare for your appointment.
Your initial evaluation is 60–90 minutes. Come with your insurance card, a list of current medications, and any questions you have. In-person or telehealth — your choice.
At the end of your first visit, you and your provider will agree on next steps. Follow-up appointments are typically scheduled before you leave.

Telehealth visits require a private, quiet space and a device with camera and microphone
We ask that you arrive 5–10 minutes early for in-person visits
Our cancellation policy requires 24 hours notice to avoid a late cancellation fee
Prescription refills are managed during scheduled appointments, not between visits

How long does the initial evaluation take?
Standard initial psychiatric evaluations are scheduled for 60 minutes. Patients with complex histories or multiple presenting concerns may be scheduled for a 90-minute comprehensive assessment instead. Please arrive a few minutes early to ensure all paperwork is complete. If you are a telehealth patient, log in to the secure video platform at least five minutes before your scheduled start time.
What should I bring to my first appointment?
Before your first appointment, you will be asked to complete intake paperwork through our patient portal, including your health history, insurance information, current medications (with doses), and a description of your primary concerns. If you have records from previous psychiatric treatment — including prior evaluations, medication lists, or therapy notes — please bring those or request that they be sent in advance. Having this information ready allows your provider to make the most of your time during the evaluation.
