We provide psychiatric and mental health services through licensed Advanced Practice Providers. Our services are intended for individuals seeking mental health care and are not a substitute for emergency services. If you are experiencing a mental health emergency, please call 988 or go to your nearest emergency room.
Appointments must be cancelled at least 24 hours in advance to avoid a late cancellation fee. Repeated no-shows or late cancellations may result in discharge from the practice. We reserve the right to decline or discontinue services at our professional discretion.
Payment is due at the time of service. We accept major credit cards, HSA, and FSA. For patients using insurance, you are responsible for any copays, deductibles, or amounts not covered by your plan.
Telehealth services are provided in accordance with applicable state laws and regulations. You are responsible for ensuring you have appropriate technology and a private space for telehealth appointments. Telehealth may not be appropriate for all conditions or situations.
To the fullest extent permitted by law, we shall not be liable for any indirect, incidental, or consequential damages arising from your use of our services. Our total liability shall not exceed the amount paid by you for services in the three months preceding the claim.
We reserve the right to modify these terms at any time. We will notify you of significant changes by posting a notice on our website. Continued use of our services after changes constitutes acceptance of the new terms.
