Please use the form below to request your first appointment with BeeWell. After you submit, you'll receive a confirmation email with a short screening form. Please book follow-ups from your patient portal so your appointment is matched to your existing chart. Booking through this form will create a duplicate record.
Sign in to the patient portal →Cancellation policy. If you need to cancel or reschedule, please give us at least 24 hours notice and there will be no charge. Cancellations less than 24 hours before the appointment, or missed appointments, are subject to a $100 fee. This fee may be waived for extenuating circumstances at the provider’s discretion. We understand life happens — just let us know.
